Q: How do I check the status of my rebate?

A: You can now check the status of your rebate by logging in to the ACUVUE^® Rebate Status page. If you provide your email address on the rebate form, we’ll let you know your submission has been received. You can expect your rebate check in about four to six weeks after you receive your email confirmation. If you have questions about your rebate, contact the ACUVUE^® Brand Rebate Center at 1-888-565-8474.

Q: Do I need to send in my original receipts with the rebate certificate?

A: No. You can submit a photocopy of your ACUVUE^® purchase receipt and eye exam receipt.

Q: Can I send in purchase receipts from multiple vendors?

A: Yes, as long as you purchase the required quantity of ACUVUE^® contact lens boxes specified on the rebate form within 90 days after your eye exam.

Q: Can I send in more than one rebate for the same purchase?

A: No. The rebate is limited to one rebate per customer, per offer, per ACUVUE^® Brand purchase, per yearly eye exam.

Q: Are ACUVUE^® OASYS^® for ASTIGMATISM and ACUVUE^® OASYS^® for PRESBYOPIA eligible for the $100 New Wearer rebate?

A:The $100 New Wearer rebate is only available for new wearers of ACUVUE^® OASYS^® Brand with HYDRACLEAR^® Plus. Wearers of ACUVUE^® OASYS^® Brand for ASTIGMATISM and ACUVUE^® OASYS^® Brand for PRESBYOPIA are not eligible for the $100 New Wearer rebate but are eligible for the $30 rebate that is offered on the entire ACUVUE^® OASYS^® Brand Family of Products .

Q: Are there any four box rebates currently available?

A: Yes. When you have an eye exam and purchase at least four boxes of ACUVUE^® OASYS^® for ASTIGMATISM you will be eligible to receive a $20 rebate.

Special note on purchases of ACUVUE^® OASYS^® Brand for ASTIGMATISM:  Eligibility for $60 rebate and $20 rebate are restricted to purchases made 9/1/09 to 12/31/09. Rebate certificates must be received at the mailing address on or before 1/31/10**. Purchases made at Costco^®, WALMART^®, Sam's Club^® , 1•800 CONTACTS^®, Sears Optical, and Target Optical and some LensCrafters^® and some Pearle Vision^® store locations are not eligible for any of these rebate offers, but other rebate offers are often available for ACUVUE^® purchases at these retailers. Please check the store for details.  All other rebate terms and conditions apply.

Rebate is in the form of a Visa prepaid card, which expires 1 year after issuance. Cards are issued by Citibank, N.A. pursuant to a license from Visa U.S.A. Inc. and managed by Citi^® Prepaid Services. Cards will not have cash access and can be used everywhere Visa debit cards are accepted.

Notice to Consumers: If you are personally filing a claim for reimbursement from a third-party payer (e.g., insurance company, employer group, etc.) for the purchase of this product, your claim must be based upon your payment less the amount of this rebate. If your doctor is filing the claim, you must notify the doctor’s office of the need to deduct this rebate amount from the purchase price used in calculating the claim.

Important notice: If you redeem for the Money-Back Guarantee, you will not be eligible to receive a rebate. If you redeem for a rebate and then redeem for the Money-Back Guarantee, you will receive the full amount of the Money-Back Guarantee minus the rebate amount already issued to you.
