How do I complete my rewards submission?

Follow the steps below:  

  1. First, create a MyACUVUE® account on acuvue.com. Then, visit the Rewards page and authenticate your rewards account using two-factor authentication. 
  2. Create a profile for Rewards and fill in all required information in the Submit Reward online form. 

 

Then, only for initial annual, 6-month and 3-month purchases at time of annual eye exam: 

  1. After submission of the online portion, you must handwrite your online confirmation code on your original receipt and mail the original receipt to: Merkle, PO Box 5085, Kalamazoo, MI 49003 5085. 
  2. When your receipt with handwritten online confirmation code is received, your submission will be processed. 
  3. If your receipt with handwritten online confirmation code is not received within 3 weeks, you will receive a rejection notification informing you that these were not received. You can always mail the receipt back in and your submission will be re-evaluated upon receipt.
What is the reward that I can earn when I purchase ACUVUE® products?

The reward you can earn is a ACUVUE® Prepaid Mastercard®. Your reward will be delivered via email with instructions for selecting either a virtual or physical card. The choice is yours!

Can I earn MyACUVUE® Rewards when I buy lenses at retailers like Costco or Walmart?

No. ACUVUE® Rewards are not valid for purchases made at retailers including (but not limited to) Costco Optical, Sam's Club Optical, BJ's Optical, Walmart Optical, Target Optical, or LensCrafters corporate locations. 

Rewards are valid for in-office purchases with participating eye care professionals, purchases made at select

retailers, and online purchases through ACUVUE® Shop or ACUVUE® Checkout.

How do MyACUVUE Rewards work?

Once you register with MyACUVUE® Rewards and submit your proof of purchase, your reward will be delivered via email with instructions for selecting either a virtual or physical Prepaid Mastercard. 

  • You must select your card type (virtual or physical) within 3 months from the date these instructions are sent via email. The link to access your payment expires after that time.  
  • Once card type is selected, the funds must be used within six (6) months or the card will expire.  
  • Use your ACUVUE® Mastercard Prepaid card anywhere Mastercard is accepted in the U.S. Issued by The Bancorp Bank, Member FDIC, pursuant to license by Mastercard International Incorporated. Mastercard is a registered trademark, and the circles design is a trademark of Mastercard International Incorporated. Your use of the prepaid card is governed by the Cardholder Agreement, and some fees may apply. This is not a gift card.
    Please note that prepaid cards are subject to expiration, so pay close attention to the expiration date of the card.

Important Information for Contact Lens Wearers: ACUVUE® Contact Lenses are available by prescription only for vision correction. An eye care professional will determine whether contact lenses are right for you. Although rare, serious eye problems can develop while wearing contact lenses. To help avoid these problems, follow the wear and replacement schedule and the lens care instructions provided by your eye doctor. Do not wear contact lenses if you have an eye infection, or experience eye discomfort, excessive tearing, vision changes, redness or other eye problems. If one of these conditions occurs, remove the lens and contact your eye doctor immediately.
